Black Creek Community Health Centre takes an innovative approach to making health care accessible to residents. Through initiatives such as the Wheels to Wellness mobile health-care unit and community ambassadors that talk to neighbours about getting checkups, the health centre is able to take care outside of its facility and to residents doorsteps.
